On test day, this will make your life easier.
The Night Before
Put everything you'll need in the car. I've made a list below.
Bring your admission ticket. Print it off from the website. They won't let you in without it.
Bring your ID (license or permit work). Here's an article for the SAT: http://sat.collegeboard.org/register/id-requirements. Here's one for the ACT: http://www.actstudent.org/faq/id.html
Bring fat, kindergarten-size pencils. You can fill in the bubbles faster this way.
Bring regular pencils for the essay and other paperwork. You'll hate writing real actual words with the kindergarten pencils. Also, some people love to bring those soft pink erasers that come separately. You're welcome to bring them; I've just personally never used them much.
Bring a calculator that you're comfortable with. A friend of mine calls this "the calculator that is your friend." Approved calculators for the SAT can be found here: http://sat.collegeboard.org/register/calculator-policy. For the PSAT/NMSQT: http://sat.collegeboard.org/register/calculator-policy. For the ACT: http://www.actstudent.org/faq/calculator.html.
Bring a snack. Salt and vinegar almonds from Publix are my favorite.
Chill. Don't attempt to take another practice test. Catch up on Downton Abbey, eat ice cream, go for a run. Treat yourself. Relax. Go to bed on time.
The Day*
Get up earlier than you think you should.
Eat breakfast. Even if you're like me and can't stand to eat something that early in the morning. Please just bring it in the car. You don't want to be so hungry you can't think.
Check in. What shakes out all the tension and lets you focus? For me, it's three slow theatre breaths.
*yes, I love Pat Frank's Alas, Babylon with all my heart.

SAT has a great article about their requirements for test day: http://sat.collegeboard.org/register/sat-test-day-simulator
Don't bring phones, iPads, laptops, etc., even as calculators. Use one of the acceptable calculators. :)
